Quick answer: Adds 2.5 damage per level to each hit on undead: zombies and their variants (drowned, husks, zombie villagers, zombified piglins, zoglins, zombie horses), skeletons and their variants (strays, bogged, wither skeletons, skeleton horses), phantoms and withers. In Bedrock the damage is rounded down.

Swords and spears at the enchanting table; axes and maces by anvil in Java, directly in Bedrock.

Best uses
Worth it for undead-heavy trips such as the Nether fortress or a wither fight. It does nothing against other mobs, so Sharpness stays the general pick.
Incompatible with
Sharpness, Bane of Arthropods, Density (mace), Breach (mace), Impaling - an item can only hold one of these.
