How to get Spooky Scary Skeleton

Find a nether fortress (see the A Terrible Fortress guide): wither skeletons spawn only inside fortresses.
Kill wither skeletons. The skull drop chance is 2.5% by default, rising to 3.5%, 4.5% and 5.5% with Looting I, II and III (same on Java and Bedrock).
Only kills by you or a tamed wolf can drop the skull, so finish them yourself.
Pick the skull up: having it in your inventory completes the advancement.

Tips

A Looting III sword cuts the average from about 40 kills per skull to about 18, so enchant before farming. Watch the Wither effect: it drains your health and turns the hearts black; milk cures it. You need three skulls to summon the wither.
