How to get Ol' Betsy

1. Get a crossbow: craft one with 3 sticks, 2 string, 1 iron ingot and 1 tripwire hook, loot one from pillager outpost or bastion chests, or take it from a pillager or crossbow piglin that drops it.
2. Carry arrows (or firework rockets) anywhere in your inventory.
3. Hold use to load the crossbow: it stays loaded until you fire.
4. Press use again to shoot. Your first shot completes the advancement.

Tips

Quick Charge III makes reloading almost instant and Multishot fires three projectiles at once; both enchantments are crossbow-only. Loading firework rockets turns the crossbow into a small grenade launcher with area damage.
